Who do we help?
Lewisham Park is a specialist scheme which offers support and accommodation to adults with ongoing mental health issues within the London Borough of Lewisham.
The service supports African-Caribbean men from 18 years of age who are experiencing housing need alongside an ongoing mental health issue. It encourages people to develop and improve their ability to live independently as this scheme is the final step before the move to independence.
About the scheme
The scheme has 6 self contained one bedroom flats with a communal garden.
We maintain good links with specialist services in the area such as the, Community Mental Health Team, Community Options, ISIS, Community Opportunities Service and the South London & Maudsley NHS Trust in order to provide extra, more specialist support.
Our scheme can support 6 people at any one time and residents stay for up to two years. At the end of this time residents are supported to make the move into independent accommodation in the local area and are offered resettlement support in order to help this transition.
We work with people to identify their needs and to find ways to achieve their goals. The support can focus on things like housing, health, money and community activities. This is agreed in a Personal Development Plan, which we regularly review with service users.
We operate from Monday to Friday, 9.00am to 5.00pm. If you receive support from us you will meet with your support worker on a regular basis, depending on your individual needs. This is worked out and agreed at the beginning of the service and regularly reviewed, however additional support is available to anyone who needs it outside these sessions.

Lewisham Mental Health Floating Support Service
Who do we help?
Lewisham Mental Health Floating Support Service is a specialist service which offers floating support and accommodation to adults with ongoing mental health issues within the London Borough of Lewisham.
The accommodation part of this service supports 12 people across Deptford, Forest Hill and Lee and the remaining capacity offers floating support to 9 people within the Lewisham area who are experiencing a range of problems accessing or maintaining accommodation in their local community.
About the scheme
Our accommodation schemes support up to 20 people in our long term accommodation with some extra capacoty to provide floating support to people once moved on from the scheme. This means that residents have an assured tenancy which gives them a level of security providing that the requirements of the tenancy are adhered to.
However, if tenants are ready to move on they are supported to make the move into independent accommodation in the local area. Everyone is offered resettlement support in order to help this transition.
The floating support element of this service can work with people for 2 years. Workers visit people in their own homes offering support and advice around housing related issues along with areas such as money, health accessing the local community.
In both cases we work with people to identify their needs and to find ways to achieve their goals. The support can focus on things like housing, health, money and community activities. This is agreed in a Personal Development Plan, which we regularly review with service users.
We can work with people regardless of what kind of accommodation they live in for example council accommodation, private rented, housing association or even if the person is homeless.
We maintain good links with specialist services in the area such as the, Community Mental Health Team, Community Options, ISIS, Community Opportunities Service, MIND and the Ladywell Unit in order to provide extra, more specialist support for both parts of this service.
We operate from Monday to Friday, 9.00am to 5.00pm. If you receive support from us you will meet with your support worker on a regular basis, depending on your individual needs. This is worked out and agreed at the beginning of the service and regularly reviewed.

Humber Road
Who do we help?
Humber Road is a specialist scheme which offers support and accommodation to adults with mental health and substance misuse issues within the London Borough of Greenwich.
The service supports adults and is aimed at people who are experiencing housing need who also have a dual diagnosis such as mental health and substance misuse, however clients referred will have completed a programme of detox or recovery. It encourages people to develop and improve their ability to live as independently as possible.
About the scheme
We can support four people at any one time and residents stay for up to two years. At the end of this period they are supported to make the move into independent accommodation and are offered up to six months of resettlement support in order to help this transition.
The scheme consists of four self contained studio flats which each have a kitchen area, bathroom and toilet. They are furnished with all the essential items people need, including a bed, cooker and fridge and there is also a communal lounge at the scheme.
Each resident at the Humber Road scheme is assigned a support worker who they meet with regularly. Outside of these support visits residents can contact In Touch Monday to Friday, 9.00am to 5.00pm to receive extra advice or support.

We maintain good links with specialist services in the area such as The Beresford Project and The Heights Mental Health Team in order to provide extra, more specialist support.
