Learning disability schemes in Kent & Medway

Who do we help?

in touch runs two specialist schemes which offer support and accommodation to adults with learning disabilities and mental health issues in Kent.

These services support adults who have learning disabilities and mental health issues and will encourage people to maintain their ability to live as independently as possible.

About the schemes

These are long term projects which support a total of 19 people in the Dartford and Herne Bay areas. The schemes consist of self contained one or two bedroom flats, some with communal areas and on site offices.

If residents would like to move on they are supported to make the move into independent accommodation in the local area and are offered a short period of resettlement support in order to help this transition.

If you receive support from us you will meet with your support worker on a regular basis, depending on your individual needs. This is agreed at the beginning of the service and regularly reviewed. Outside of these support visits, residents can contact in touch Monday to Friday, 9.00am to 5.00pm to receive extra advice or support.

We maintain good links with specialist services in the area such as the Community Mental Health and Learning Disability teams at the local Primary Care Trusts in order in order to provide extra, more specialist support.

We work with people to identify their needs and to find ways to achieve their goals. The support can focus on things like housing, health, money and community activities. This is agreed in a Personal Development Plan, which we regularly review with service users.

What next?

The support received from in touch will not cost you anything if you are receiving benefits as it is paid for by the Supporting People Team at the Borough Council.

Depending on the particular scheme, referrals to this scheme are made through Dartford Borough Council or Canterbury Social Services.
