Who do we help?
The East Hants Floating Support Service offers support to people living within the East Hampshire District Council area.
The service supports people over the age of 16 and is aimed at those who are experiencing a range of problems accessing or maintaining accommodation in their local community. It supports service users who want to develop and improve the ability to live independently.
About the service
Our service supports up to 62 people at any one time and lasts for no more than two years. We operate a waiting list for this service which is run on a priority need basis.
We operate from Monday to Friday, 9.00am to 5.00pm. If you receive support from us you will meet with your support worker on a regular basis, depending on the individuals needs. This is agreed at the beginning of the service and regularly reviewed.
We work with people to identify their needs and to find ways to achieve their goals. The support can focus on things like housing, health, money and community activities. This is agreed in a Personal Development Plan, which we regularly review with service users.
We can work with people regardless of what kind of accommodation they live in for example council accommodation, private rented, housing association or even if the person is homeless.

You will not have to pay anything for this service as it is paid for by the Supporting People Team at the East Hampshire District Council.

New Forest Floating Support Service

The New Forest Floating Support Service offers support to people living within the New Forest area.
The service supports people over the age of 16 and is aimed at those who are experiencing a range of problems accessing or maintaining accommodation in their local community. It also supports service users to develop and improve their ability to live independently.
About this service
Our service supports up to 60 people at any one time and is a short term service of up to two years. We have a waiting list but people are given priority need if there is a risk of immediate loss of their accommodation.
We operate from Monday to Friday, 9.00am to 5.00pm. If you receive support from us you will meet with your support worker on a regular basis, how often will depend on your own individual needs. This is worked out at the beginning of the service and regularly reviewed.
We work with people to identify their needs and to find ways to achieve their goals. The support can focus on things like housing, health, money and community activities. This is agreed in a Personal Development Plan, which we regularly review with service users.
We can work with people regardless of what kind of accommodation they live in for example council accommodation, private rented, housing association or even if the person is homeless.

New Forest Resettlement
Who do we help?
The New Forest Resettlement Service offers support to people living within the New Forest District Council area.
The service supports young single people under the age of 25 and is aimed at those moving on from one of our specialist schemes into independent accommodation. The service supports people to continue to develop and improve their ability to live independently following a period of living within a more supported environment.
About this service
Our service supports up to five people at any one time and lasts for a maximum of two years.
We operate from Monday to Friday, 9.00am to 5.00pm. If you receive support from us you will meet with your support worker on a regular basis, how often will depend on your own individual needs. This is worked out at the beginning of the service and regularly reviewed.
Support workers can help with a wide range of areas including things like housing, health & money, resulting in completing an individual Personal Development Plans based on how the service user feels and what areas they would like support with.
We can work with people regardless of what kind of accommodation they live in for example council accommodation, private rented, housing association or even if the person is homeless.

You will not have to pay anything for this service as it is paid for by the Supporting People Team at the New Forest District Council.

The Rushmoor and Hart Floating Support Service offers support to people living within the areas of Rushmoor Borough Council (Aldershot, Farnborough and part of Ash) and Hart District Council (Odiham, Hook, Fleet, Hartley Witney, Yateley and Blackwater).
The service supports people over the age of 16 and who are experiencing a range of problems accessing or maintaining accommodation in their local community. It also supports service users to develop and improve their ability to live independently.

Our service supports up to 134 people at any one time and lasts for a maximum of two years. We operate a waiting list for this service which is run on a priority need basis.
We operate from Monday to Friday, 9.00am to 5.00pm. If you receive support from us you will meet with your support worker on a regular basis, how often will depend on your own individual needs. This is worked out at the beginning of the service and regularly reviewed.

Rushmoor and Hart Family Intervention Project
Who do we help?
The Rushmoor and Hart Family Intervention Project Service offers support to people living within the areas of Rushmoor Borough Council (Aldershot, Farnborough and part of Ash) and Hart District Council (Odiham, Hook, Fleet, Hartley Witney, Yateley and Blackwater).
The service supports families that are at immediate risk of eviction and homelessnessas a result of chaotic or anti social behaviour.
About the service
Our service supports families to prevent imminent eviction, providing up to 10 hours family support each week for a maximum of two years. We operate a waiting list for this service which is run on a priority need basis.
If you receive support from us you will meet with your key worker on a regular basis, how often will depend on your own individual needs. This is worked out at the beginning of the service and regularly reviewed.
We work with people to identify their needs and to find ways to achieve their goals. The support can focus on things like housing, health, education, employment, money and community activities. Your key worker will work with all family members as appropriate and dependent on need.
We can work with people regardless of what kind of accommodation they live in for example council accommodation, private rented or housing association.

Southampton
Homeless Floating Support Service
Who do we help?
The Homeless Floating Support Service offers support to people living within Southampton.
The service supports adults between the ages of 16 and 60 and is aimed at individuals and families who are at the risk of being made homeless and who could be experiencing a range of problems accessing or maintaining accommodation in their local community. It also supports people who want to develop and improve their ability to live independently.
About the service
Our service supports up to 220 people at any one time for a maximum of two years. We operate a waiting list for this service which is run on a priority need basis.
We operate from Monday to Friday, 9.00am to 5.00pm. If you receive support from us you will meet with your support worker on a regular basis, depending on you individual needs. This is worked out and agreed at the beginning of the service and regularly reviewed.
We work with people to identify their needs and to find ways to achieve their goals. The support can focus on things like housing, health, money and community activities. This is agreed in a Personal Development Plan, which we regularly review with service users.
Remember you do not have to be a rough sleeper to be considered homeless; we help all sorts of people in many different living arrangements.

Intensive Homeless Service
Who do we help?
The Intensive Homeless Service is a specialist scheme in Southampton which offers support and accommodation to vulnerable homeless adults with high support needs within the Southampton City Council area.
The service supports adults between 18 and 60 years old and is for people who are experiencing housing need alongside other support needs. We support service users to develop and improve their ability to live independently.
About the service
Our scheme can support 39 people at any one time and residents stay between six and nine months. At the end of this residents are supported to make the move into more independent accommodation in the local area.
The scheme consists of 26 one-bedroom flats, one five-bedroom shared house and one eight-bedroom shared house.
Each resident at the scheme is assigned a support worker who will meet with regularly with their clients.
We work with people to identify their needs and to find ways to achieve their goals. The support can focus on things like housing, health, money and community activities. This is agreed in a Personal Development Plan, which we regularly review with service users.
For customers assessed as requiring an additional care services along with the support offered at the scheme, we are able to provide this in house via the In Touch Hampshire Domiciliary Care Service. This ensures a joined up and flexible approach which helps to establish a robust support & care package which can be continued after move on.
Within the service we offer a wide range of meanigful activities which encourage customers to get involved and learn new skills. These skills can contribute to customer overall sense of confidence and support their eventual move on from the service.
What next?
The support received will not cost you anything as it is funded by the Supporting People Team at Southampton City Council.
