Hamwic House

Who do we help?

Hamwic House is a specialist scheme which offers support and accommodation to older people in the City of Southampton.

This service support adults over the age of 55 with support needs and encourages people to maintain and develop their ability to live as independently as possible.

About the scheme

Hamwic House is a long term service that supports a total of 35 people. The scheme consists of 35 self contained flats with a communal lounge, kitchen, garden and a small car park.

Hamwic House is an independent living scheme, however extra support or care can be arranged from outside agencies if required. If this extra support is needed referrals can be made by an in touch staff member.

Residents have an allocated worker but can also contact In Touch at the main office Monday to Friday, 9.00am to 5.00pm to receive extra advice or support.

We maintain good links with specialist services in the area such as Community Nurses, chiropodists, district nurses, occupational therapists, MIND, Social Services and the Community Mental Health Service in order to provide extra, more specialist support.

We work with people to identify their needs and to find ways to achieve their goals. The support can focus on things like housing, health, money and extra care or services that may be needed. This is agreed in an Independence Plan, which we regularly review with service users.

What next?

Depending on your circumstances the support received from in touch may not cost you anything as it is funded by the Supporting People Team at the Southampton City Council.

Cranbury Towers

Who do we help?

Cranbury Towers is a specialist scheme which offer support and accommodation to older people in Southampton.

This service is for adults over the age of 55 with various support needs such as literacy or memory problems. We encourage people to maintain and develop their ability to live as independently as possible.

About this scheme

Cranbury Towers is a long term service and supports a total of 10 people. The scheme consists of 10 self contained flats with a communal lounge, kitchen and a garage. Each flat also has a 24 hour intercom system connected to the Invicta Telecare contact centre, in case of emergencies.

Cranbury Towers is an independent living scheme, however extra support or care can be arranged from outside agencies if required. If this extra support is needed referrals can be made by an In Touch staff member.

Older Person Floating Support

Who do we help?

The Older Persons Floating Support Service offers support to people living within the area served by Southampton City Council.

The service supports adults aged 60 and over with support needs which may include issues like alcohol dependency, mental health problems, memory problems and so on.

About this service

Our service supports up to 39 people at any one time for up to 2 years. We operate a waiting list for this service which is run on a priority need basis.

We operate from Monday to Friday, 9.00am to 5.00pm and if you receive support from us you will meet with your support worker on a regular basis. How often this is will depend on your individual needs. This is worked out and agreed at the beginning of the service and regularly reviewed.

The service is aimed at individuals at risk of being made homeless and who could be experiencing a range of problems accessing or maintaining accommodation in their local community. It also encourages people to maintain and develop their ability to live independently.

We work with people to identify their needs and to find ways to achieve their goals. The support can focus on things like housing, health, money and extra care or services that may be needed. This is agreed in an Independence Plan, which we regularly review with service users.

We can work with people regardless of what kind of accommodation they live in for example council accommodation, private rented, housing association or even if the person is homeless.

What next?

You will not have to pay anything for this service as it is funded by the Supporting People Team at Southampton City Council.

If you feel that you would benefit from this kind of service you can apply through the Local Authority, other local statutory and voluntary agencies, GP’s or local housing associations.

Hampshire Domiciliary Care Services

Who do we help?
The Hampshire Domiciliary Care Service offers support to people living within the area served by Southampton City Council, including Lordshill to Hedge End, Shirley and Bitterne.

The service provides care to adults aged over 18 who have learning disabilities and physical disabilities. This care is provided in your own home and enables you to maintain your ability to live as independently as possible.

About this service

This service currently provides support to 30 people.

We operate 7 days a week, 365 days a year, Monday to Sunday from 7am to 10pm including some overnight support.

We can work with you regardless of what kind of accommodation you live in i.e. council accommodation, private rented, housing association etc.  Your care will be paid by Southampton City Council or personally depending on if your care manager commissions our service or if you commission our service directly.

We are a registered Care Service with The Care Quality Commission and have been awarded a good rating in our most recent inspection.
